Please note, administrative work is done outside office hours. For urgent requests, contact your medical doctor.

General document fees

$20 Note for work or school (1 page) (2-3 days)

$35 and up Release of file to patient (1 to 2 weeks)

$50 Narrative report and assessments after initial visit (2 weeks)

$150 Narrative report, findings and recommendations for patient, lawyer, insurance or other  (2-3 weeks)

MVA Forms

These fees *should* be reimbursed by the insurance, please double check with your adjuster

$200 OCF-18

$150 All other OCF forms

$150 Release of files for MVA
